Required information
- Identification of the copyrighted work claimed to have been infringed.
- Identification of the material that you believe is infringing, with enough detail to locate it.
- Your contact information (name, address, telephone, email).
- A statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent, or the law.
- A statement, under penalty of perjury, that the information is accurate and that you are the copyright owner or authorized to act on the owner's behalf.
- Your physical or electronic signature.

If you believe material was removed in error, you may submit a counter-notice with the information specified by 17 U.S.C. § 512(g)(3).
